In order to achieve optimal effects, skin contact is important, so if one has problems with one's back then it should not hang too loosely. If one has lumbar back problems then it is better to use the back brace. An alternative is to sleep in the T-shirt as that provides good skin contact. If it's the shoulders one has problems with then the T-shirt may be looser fitting as the T-shirt always has direct contact with the shoulders.
